What are the most critical factors when considering Extreme Air Coolers?

I am just curious, other than price of course since Extreme Air Coolers are not cheap normally

There is nothing extreme about air coolers, unless you take your rig outside, or use a duct to the outside. Assuming it's winter out there.

"Extreme air coolers" uses other technologies so they are not only air coolers. Example, the V12 from CM uses something else I cant remember (TEC?)

Yep, the V12 is a TEC/air cooler combo. The TEC cools the CPU, while the air cooler cools the TEC.

I don't think he was asking for a definition of extreme.

Well of course besides price you would want to consider:
What are you going to do with your rig? Are you trying to get the highest possible overclock while still using air? Will it fit in your case? Will it interfere with the ram. Some of the best air coolers are huge.

For example a Megahalems or Noctua NH-D14 will interfere with the first ram slot if you are using OCZ Reaper ram or many others with tall heat sinks on some/most motherobards.

I blow air to my CPU with my mouth. Is that extreme enough?

Depends how hard you can blow, 10psi would be considered extreme. :P
